A case management system is built with the intent to foster cross discipline interaction but staff use the tool with the silo based mindset. Information that could be shared isn’t shared, but the system is being used, so deemed to be a successful implementation.
This was what I saw and felt during my first social work placement after having come from the programming world. Case notes taking up a large part of the working day, people 6 months behind on going from notebook to digital, never to see the light of day again unless something bad happened. Structured enough to give an outline of an individual, but not enough to become a datapoint in something larger.
Facebook was about 10 years old, and everyone knew about sharing data with some people and not others, moving things around quickly. But in the health world, data felt “gluggy”, it didn’t move, it didn’t inform. It sat and waited. I thought semantic tagging was the way forward, with ICD or similarly standardised codes. A good meta data schema. Implementing it may not be that hard, but creating good data seems pointless if it is going to be put into a gluggy system.
Part of the hope for AI is that it could be used to go through old case notes, attend to systematic semantic tagging, and help find patterns that can be used in research.
